BlueCross building medical home concept
BlueCross BlueShield of Tennessee is continuing to build its patient-centered medical home strategy by deploying IT systems to some of its local providers. The SmartTransitions system from Performance Clinical Systems, first installed for Holston Medical Group in Kingsport, helps physicians create patient discharge plans and follow-up with patients to help reduce readmissions."With recent studies showing readmission rates as high as 20 percent within 30 days of leaving the hospital and with our associated costs for these readmissions being as much as $60 million a year, we believe creating a link between inpatient and outpatient care teams is critical for improving outcomes and satisfaction for our members, as well as for those who pay the bill," said Dr. Robert Mandel, senior vice president of health care services for BlueCross. "We aim to empower all participants in the transition of care workflow with an easily embraced solution that not only supports evidence-based medicine, but makes care related information accessible in a much more timely manner than traditional processes."

Feeling motivated yet?
BlueCross BlueShield of Tennessee has launched a Pandora radio station with tunes you can work out to — including an option for moving at 100 percent exertion.“We want, not only our members, but all Tennesseans to get the most out of their workouts, whether it’s at the gym, in the yard or spending time with their families,” said Janet McConnell, director of brand strategy and new media for BlueCross. “Music helps us get in the mood to work out and motivates us to keep going when we feel like quitting.”Pandora is an Internet radio service that uses the Music Genome Project letting users create customized stations based on the artists, songs and albums they like while weeding out what they don't like.
